Millions of women around the world struggle with stubborn lower-body fat that doesn’t respond to dieting, clean eating, or regular workouts. In India, the challenge is even greater due to limited awareness and delayed diagnosis. They bruise easily. Their legs feel heavy, painful, and swollen. And worse, they’re often told to “just lose weight”, “go on a diet”, “be more active” and so on.
But what if it’s not regular fat?
What if it’s Lipedema, a misdiagnosed medical condition that has nothing to do with overeating or inactivity?
Let’s break down the truth about lipedema: what it is, how it differs from obesity, and why diet and exercise don’t work.
Lipedema is a chronic fat and connective tissue disorder that mostly affects women. It causes unusual fat growth in an asymmetric way, mainly in the hips, thighs, buttocks, legs, and sometimes arms.
It’s not just about appearance. Lipedema fat is painful, inflamed, and resistant to diet or exercise. Over time, it can cause mobility issues, joint damage, and emotional distress. Yet many people still confuse it with simple obesity or lymphedema.

Obesity is a general increase in fat throughout the body due to excess caloric intake and low activity. It can respond well to lifestyle changes, diet, and exercise.
Lipedema fat is different.
Women with lipedema often say:
“I lost weight from my face and waist — but my legs stayed the same.”
Recognizing this difference is crucial, because the wrong diagnosis leads to the wrong treatment and unnecessary shame.
One of the most disheartening realities for people with lipedema is that even the cleanest diet and the best workout won’t shrink lipedema fat. Here’s why:
Lipedema tends to emerge or worsen during times of hormonal fluctuation. Estrogen appears to play a role, which is why it affects women disproportionately and clusters in families.
Unlike normal fat, lipedema tissue is chronically inflamed. This leads to fibrosis — thick, rope-like tissue that becomes even more resistant to breakdown.
Many lipedema patients experience lymphatic congestion, where the body can’t properly drain fluid. This causes swelling and further tissue expansion, especially in the legs.
Lipedema fat has more alpha-adrenergic receptors. These stop fat burning. It has fewer beta receptors, which help break down fat. This means the body won’t “choose” to burn lipedema fat during weight loss.
Lipedema fat cells are larger, more numerous, and surrounded by fluid and dysfunctional capillaries. It behaves more like scar tissue than energy storage.
If you have lipedema, no amount of dieting or cardio will “fix” the fat — because it’s not regular fat.

Here are common signs and symptoms that may point to lipedema rather than typical weight gain:
Symptom | Lipedema | Obesity |
Fat distribution | Symmetrical, legs/arms only | Generalized across body |
Hands and feet | Spared | Can be affected |
Pain | Common (burning, aching, tender) | Uncommon |
Easy bruising | Yes | Rare |
Texture | Nodular, fibrotic, swollen | Soft, even |
Weight loss effect | Upper body shrinks, lower body unchanged | Uniform fat loss |
Swelling | Increases during day | Minimal or activity-related |
If this sounds like you or someone you know, especially if symptoms started around puberty or pregnancy, consider talking to a trained specialist about lipedema.
These focus on managing symptoms, not removing the fat:
Limitations: These methods can slow progression and improve comfort, but cannot remove the diseased fat.
The only treatment that can permanently remove lipedema fat is lymph-sparing liposuction, done by a trained and experienced surgeon. It’s a medical, not just cosmetic, procedure.
Liposuction for lipedema is not the same as standard cosmetic lipo. It requires specific techniques to protect lymph vessels and manage inflamed tissue.

In India, lipedema is rarely diagnosed and often misinterpreted as obesity or lymphedema. But change is beginning.
Lipedema care in India is growing. Leading this effort is Dr. V.K. Srinagesh, a Senior Consultant Plastic & Reconstructive Surgeon at Omega Hospitals in Hyderabad.
With over two decades of surgical expertise, Dr. Srinagesh is one of the first doctors in India to formally recognize, diagnose, and treat lipedema as a distinct medical condition, rather than confuse it with obesity or lifestyle-related fat gain.
“Lipedema isn’t your fault. And with the right treatment, it doesn’t have to control your life.”
